Vertebrogenic pain

Pain associated with vertebral endplate changes. MRI findings must fit the clinical picture.

What it can mean

Deep, central low back pain can sometimes be associated with damage or inflammation at the vertebral endplates. Sitting, bending, or lifting may aggravate symptoms, but these features overlap with other causes.

How it is evaluated

MRI may show Modic type 1 or type 2 endplate changes. These findings must be interpreted with the history and examination; they do not prove the pain source by themselves.

How care is planned

Conservative care is reviewed first. Basivertebral nerve ablation may be considered when symptoms, imaging, and treatment history meet the relevant selection criteria.

Your diagnosis depends on an individual evaluation. Several pain sources can overlap.
